To be eligible for an account with Practitioner Supplies you must be in business that is in retailing the products you are buying or in a business that uses the products to service the public. If you have any questions about whether or not you qualify for an account please contact us.

To apply for a wholesale account please fill out the submit the form below.

Please be sure to provide us with a valid email address so that we can notify you once your account has been approved and activated. We aim to approve accounts within 24 hours of application.
